Case Management Department Mission Statement
The Case Management Department of Newton Medical Center will strive to coordinate appropriate, quality health care services, while promoting the delivery of cost-effective services that meet patient, provider and payer demands. Case Management Services are available to all patients while at Newton Medical Center. A case manager or social worker will assist with planning for discharge, verifying insurance coverage and finding resources.
Some routine services provided:
- Meals on Wheels
- Home Health and/or Hospice services
- Nursing home returns or coordinating new placement at a nursing home
- Verifying insurance coverage for walkers, oxygen, nebulizers, etc.
